A life stage transition is a significant change or shift in the circumstances and support needs of an individual with a disability. Each requires different types and levels of care and support. These transitions are common for people with disabilities and may involve profound changes in living arrangements, education, employment, healthcare, and social activities. It will not be unusual for families to opt for respite services to help them along.

As an NDIS provider in Victoria, we’ve assisted people with disabilities from a wide range of life stage transitions which can include the following:

  • Childhood to Adulthood
This transition occurs when a child with a disability reaches the age of adulthood. It involves preparing the disabled individual with life skills for greater independence. This requires adjustments to meet their changing social support needs.
  • Leaving Home and Participating in Society
Our community is where we belong and whatever our capabilities are, there are never-ending ways to contribute and participate in our society. But people with disabilities may face unique challenges when it comes to this. They will need the appropriate care and support for them in the new living environment.
  • Employment Transitions
Some people may need job accommodations or modifications to enable them to work in a safe and productive environment. They may need additional training or education to be able to obtain employment or help them transition into the workforce.

We enable persons with disabilities to pursue and accomplish their goals while participating in community activities fostering the acquisition of new skills. For our valued NDIS participants, we offer them our customized support that aims to elevate their quality of life during these transitions.

Disability Diversity Care (DD Care) is a registered NDIS provider offering unique and innovative disability care in Melbourne, Victoria suited for life stage transitions.
